The Nutrition Assessor for Greater Boston Chinese Golden Age Center (GBCGAC) is to provide nutrition assessment screening to determine eligibility, social services referrals, and community outreach to elders and their families.

DUTIES AND RESPONSIBILITIES

  • Initiate home delivered meals and process changes in participant status as a result of reassessments.
  • Conduct in-home assessments of potential clients to determine their eligibility for home delivered meals.
  • Conduct reassessments as required of home delivered meals clients to determine their continued eligibility for any additional nutrition services.
  • Maintain HDM client files including HDM meals form, reassessment forms, and route sheets.
  • Record and update appropriate database on Title IIIC HDM clients.
  • Refer clients to appropriate agencies and community resources and provide follow-up to determine if contact was made and needs are met.
  • Report and review nutrition issues with Dietitian.
  • Prepare monthly statistics and billing.
  • Assist in outreach, activities, such as vendor fairs, community events, and Farmer’s market coupon distribution.
  • Assist in daily QC on Home delivered Meals.
  • Provide phone coverage and other administrative support to nutrition program.
  • Perform other related duties as assigned by supervisor.
  • This job reports to Dietitian.

Qualifications

  • One year of experience in human services working with elders; experience doing assessments preferred.
  • Be sensitive to needs of the elders and work with team members effectively in the multi-cultural setting.
  • Microsoft Office experience required.
  • Traveling between locations necessary.
  • Excellent organizational skills.
  • Cantonese and Mandarin Mandatory. English preferred.
