Adult Day Health Care is a day program that
aims to provide cost-effective health care and support services to frail elders and
disabled adults in a structured and supervised environment. Eligibility is based on
an evaluation by the program staff on an individual basis. Professional bilingual staff provides skilled
nursing care, health monitoring, nutritional services, health and public education, social
service, information and assistance, coordination of services, caregiver support,
recreational and educational activities, and transportation. Adult Day Health Programs are operated at
three different sites: Quincy Tower, Hong Lok House, and Brighton House. The goals of this program are to improve the
physical and emotional well-being of clients, to support and enhance our clients' quality
of life and ability to live independently in the community, to prevent isolation due to
age, physical impairments, or linguistic and cultural barriers by promoting socialization,
and to provide support to caregivers. Adult Day Health Care benefits the client,
the family, and the community. For the client, ADH prevents or postpones
institutionalization, strives to restore or maintain physical and emotional well-being,
promotes and maintains independence, offers individualized and culturally sensitive plan
of care, and provides social and recreational activities. For the family, ADH provides counseling and
case management services, offers respite, support, and education, delivers cost-effective
quality care, and enables family members to continue working while Adult Day Health cares
for clients. Finally, for the community, ADH serves as an
integral part of the community service network and long-term care continuum while
providing low-cost quality care.
